Advantage Care Health Centers

Advantage Care Health Centers is a non-profit, community-based healthcare organization with two locations on Long Island, in Freeport and Brookville. It operates as a Federally Qualified Health Center (FQHC) and is also recognized as a "Patient-Centered Medical Home Level 3," which signifies its commitment to a coordinated and patient-centered approach to care.
