Community Health and Dental Care

Community Health and Dental Care (CHDC) is a non-profit organization dedicated to providing accessible and Affordable Healthcare services to communities in southeastern Berks, northern Chester, and western Montgomery counties in Pennsylvania. The Barto Office, located at 1315 PA-100, Barto, PA 19504, is one of several conveniently located health centers operated by CHDC, with other locations in Pottstown and Boyertown. Since its establishment in 2008, CHDC has focused on ensuring that all individuals in its service area have access to appropriate levels of care, regardless of their ability to pay. As a Federally Qualified Health Center (FQHC) since 2012, CHDC receives federal funding that enables it to offer a healthcare discount program based on family size and household income, providing discounts of up to 100% for eligible uninsured and underinsured patients. They also accept most major commercial insurance plans, Medicare, and Medicaid. The Barto Office functions as a comprehensive "medical and dental home," offering a wide range of integrated services under one roof. These services include: Medical Care: Comprehensive primary medical care for adults and children, including well visits, sick visits, physicals (school and sports), and immunizations. They also provide chronic care management for conditions like diabetes and high blood pressure. Dental Care: Full-service family dental care for all ages, encompassing preventive care (checkups, cleanings, sealants, night guards), restorative treatments (fillings, crowns, root canals), and advanced dental care (extractions, urgent issues). Pediatric dental services are also a key offering. Behavioral Health: Comprehensive behavioral health and psychiatry services for patients of all ages, including one-on-one counseling sessions, substance abuse treatment, and referrals for long-term care. These services are integrated with Primary Care to provide holistic support. Physical Therapy: Rehabilitation and pain management services for injuries or chronic conditions. Other Services: CHDC also offers OB/GYN, vision, podiatry, medication-assisted treatment (MAT), care and case management, and even free transportation to and from all CHDC health center locations to remove barriers to care.
