Detailed Introduction

The Cordelia Martin Community Health Center is a vital healthcare institution located at 615 Division St, Toledo, OH 43604. It operates as a Federally Qualified Health Center (FQHC) and is Medicare Certified, playing a crucial role as a safety-net provider in the Toledo area. Established in 1969 by a group of dedicated community residents, spearheaded by Mrs. Cordelia Martin herself, the center's founding mission was to address the critical need for healthcare access among inner-city residents who faced financial barriers to medical care. As part of the Neighborhood Health Association (NHA), Cordelia Martin Community Health Center is committed to putting health within reach of all community members, irrespective of their ability to pay. The center accepts most insurance plans, including Medicaid and Medicare, and offers a sliding fee scale for uninsured or underinsured patients, ensuring that no one is turned away due to financial constraints. Services provided at Cordelia Martin Community Health Center are extensive and designed to meet the diverse needs of the community. These include adult primary care, cardiology services (available to all NHA patients through their Nexus location), laboratory services, HIV testing and counseling, and various immunizations like tetanus and flu shots. The center also provides free blood sugar and blood pressure checks, diabetes education classes, free pregnancy tests, and family planning services. Additionally, they offer sports and employment physicals, mental health support, podiatry, and a 24-hour nurse triage line for urgent concerns. The WIC (Women, Infants, and Children) program is also available to support mothers and young children with nutritional assistance. Through its social services department, the center offers prescription and referral assistance. The center's commitment to community well-being extends beyond direct medical care, fostering an environment where individuals are empowered to take control of their health. It reflects the broader community health center movement's dedication to equity and community empowerment, originating from the civil rights struggles of the 1960s.
