Cornell Scott - Hill Health Center (CS-HHC) is a prominent Federally Qualified
community health center (FQHC) that has been serving communities in
Connecticut since 1968. It was founded through a partnership between members of the Hill Parents Association and Mr. Cornell Scott, in collaboration with the Yale School of Medicine. The center's mission is to measurably improve the health and well-being of the communities it serves by providing excellent, compassionate, and accessible care.
CS-HHC operates as a private,
non-profit 501(c)(3) organization and is accredited by The Joint Commission. It is dedicated to providing integrated healthcare services, including medical,
dental, and
behavioral health, across multiple sites throughout the Greater
New Haven and Lower Naugatuck Valley areas. The center provides
primary care (pediatrics, internal medicine, obstetrics/gynecology), as well as a wide array of specialty services such as allergy, audiology, cardiology, dermatology, gastroenterology, infectious disease, ophthalmology, orthopedics, podiatry, and more.
A core principle of the health center is its commitment to serving everyone in the community, regardless of their ability to pay. It accepts Medicare, Medicaid, and most commercial insurance plans, and also offers a sliding fee discount program for qualifying individuals based on income and family size. The center also participates in the Federal 340B drug program, which helps provide reduced-price medications to uninsured patients.
With over 20 clinical sites and school-based health centers, Cornell Scott - Hill Health Center serves more than 55,000 patients annually. The center is also recognized for its dedication to patient-centered care, having been proclaimed a Patient-Centered Medical Home (PCMH) by The Joint Commission. It is a national leader in community healthcare, consistently working to address health disparities and provide culturally competent care to underserved populations.
