Detailed Introduction

Crescent Community Health Center (Crescent CHC) is a well-established, non-profit community health clinic located in Dubuque, Iowa, specifically at 1690 Elm St #300. It was founded in October 2006 by a dedicated group of community members who recognized the critical need for accessible primary medical and dental care for underserved populations in the area. Crescent CHC's mission is to provide high-quality, respectful, affordable, and comprehensive care to improve the health and well-being of the communities it serves. Their vision is to achieve "quality care for all resulting in healthy communities." They operate on a philosophy of patient-centric care, partnering with individuals in their treatment plans and emphasizing the dignity of each person. The center offers a wide range of services, including: Primary Medical Care: This encompasses routine check-ups, chronic disease management (for conditions like diabetes, hypertension, asthma, etc.), acute visits, and well-child exams. They employ a care team approach with physicians, nurse practitioners, nursing staff, pharmacists, care coordinators, behavioral health specialists, and health educators. Dental Care: Services include routine cleanings, fillings, extractions, and other primary dental care. Behavioral Health: They provide counseling and therapy services, as well as psychiatric care and medication management. Wellness Programs: These programs aim to promote overall health and well-being, including a wellness center to help patients achieve fitness goals. Pharmacy Services: Crescent CHC has expanded its services to include an in-house pharmacy, enhancing medication access and management for their patients. Specialty Referrals: While focusing on primary care, they also provide referrals to community providers for specialty care when needed. Crescent CHC is certified as a Patient-Centered Medical Home by the NCQA, demonstrating their commitment to coordinated and comprehensive care. They are dedicated to accessibility, offering services to people of all ages, from any region, regardless of insurance status. For uninsured patients, the cost of services is determined on a sliding fee scale, discounted based on household income and size. The organization has a strong track record of community impact, having served thousands of patients annually (e.g., over 9,248 patients in 2022 with more than 33,106 clinic visits). They actively engage with the community through partnerships and outreach programs, addressing social determinants of health and striving to reduce healthcare disparities. Crescent CHC has received numerous awards and recognitions for its quality of care and community contributions, including multiple Health Center Quality Leader awards from the Health Resources and Services Administration (HRSA). I took my daughter here for her first dental appointment. The dentist was nice, the dental assistants were nice, and I felt like we had a good experience. Flash forward to now, I call to make an appointment for myself. The the people who answer the phone are straight up rude. I almost hung up after the first thing out of their mouth. I won’t be going back nor will my daughter.

Wonderful place! Friendly helpful staff. Dr. Nicole Kruze is magnificent! I felt like she listened and heard me. Remarkable feeling when you leave the doctors office knowing all your concerns were addressed. She was down to earth great sense of humor and yet highly intelligent.

I didn't feel belittled or rushed. I do not typically take the time for review writing but this was noteworthy. I would recommend this establishment and Dr Kruze to anyone looking for a primary care physician.

I highly recommend this dental clinic! The entire team was amazing to my brother who is intellectually disabled, scared of dentists, suffered from anxiety, and is terrified of needles. We have no complaints whatsoever. From checking in with Lisa and Selina, to the the cheerful (and fast moving) X-ray tech Susan, to the amazing Cathy that recognized my brother from working with him in the past and made him feel comfortable, to the best dentist ever for Travis, Dr.

G who explained every single thing to Travis and named her tools and was able to really relate to him and make him as comfortable as possible. We thank you all so very much!!

This is in reference to the health center/ fitness center. You need to train your staff in customer relations. The dark haired lady is rude. Not friendly at all. Took my sister down there to show it to her. She's a patient at Crescent. She was told she needed a docs release. Fine, I understand that. Asked her if she could just look. A rude no, that can be later. Then I asked for a schedule.

Again, no. She gets that later. I told her the schedule was for me. As I already go there. But not anymore. I felt embarrassed for showing this place to my sister. After we left my sister said she would never go there after the way we were treated. So I told my sister. Neither will i.
