Detailed Introduction

Crusader Community Health is a prominent non-profit organization established in 1972, operating as a Federally Qualified Health Center (FQHC) in the Rock River Valley area of Northern Illinois. Its core mission is to provide high-quality, comprehensive, and affordable primary health care to all people in need, with a particular focus on serving medically underserved individuals. The organization offers a wide array of services including adult and family primary care, pediatrics, women's health (including prenatal and pregnancy care), dental services, optical services, pharmacy services, and behavioral health (mental health and substance use) care. They also run specialized programs such as Healthcare for the Homeless (now known as Community Connections), which provides comprehensive medical care and linkage to social services for homeless individuals, and a Living with HIV/AIDS program. Crusader Community Health emphasizes health education and disease prevention, offering case management for chronic diseases like diabetes and asthma. Crusader Community Health operates multiple clinic sites, including school-based health centers, and works collaboratively with other community agencies to ensure widespread access to healthcare across Winnebago and Boone Counties. The organization accepts various insurance plans, including Medicaid and Medicare, and offers a sliding scale fee system for uninsured or underinsured patients to ensure affordability. The history of Crusader Community Health dates back to 1966 with the establishment of a well-baby clinic by Sister Catherine Francis. The need for preventative pediatric care grew, leading to the chartering of Crusader Community Health as a nonprofit in 1970, and the opening of its first official clinic in 1972. The organization has consistently expanded its services and locations, including the Broadway clinic (address provided) which opened in 1997 as its second site. The Crusader Community Health Foundation, formed in 1985, provides crucial financial support for the clinic's growth and programs. Crusader Community Health is deeply embedded in the community, working to eliminate disparities in healthcare and champion the needs of the underserved. They have been recognized for their contributions, including the annual "Spirit of Caring" awards which acknowledge outstanding volunteer achievement and leadership within the community. Their commitment extends to improving the surrounding neighborhoods, as demonstrated by initiatives to address blight near their clinics. Annually, Crusader Community Health serves over 42,000 individuals, accounting for more than 150,000 patient visits, with a significant portion of their patients having incomes at or below 200% of the federal poverty level.

I saw Dr. Griffith for the first time today. He was personable, kind, and seemed genuinely interested in helping me with my concerns. He also listened well and worked with me when I was concerned about starting a new medication. I'm very grateful to have a PCP who practices patient-centered care! The rest of the staff were supportive and helpful as well.

I came for annual physical which is free because it is covered by my insurance provider. They used a wrong coding and sent me medical bills. I just asked them to do A1C test instead of blood sugar test cause it is more accurate. They wrote that the reason for visiting was not an annual physical exam but "ongoing concerns with possible DM" which is lie.

They refused to change coding to charge my insurance provider. I will not go this clinic again.

I had an appointment today with the obgyn al khabbaz after having an abortion due to health issues and me and my fiancé having financial issues, I didn’t want to bring another child into this world at the moment knowing we don’t have the stability and he made a very disrespectful comment he said “so your plan is to have unprotected sex and have another abortion” and that comment literally hurt me so bad because I’m literally on birth control and engaged and he still attempted to do a pelvic ultrasound on me and I told him NO! I started to cry as I made a complaint against him. A man with that type of mindset should NEVER TOUCH A WOMAN! I definitely didn’t find that funny as a joke because that’s something I will forever have to hold in my heart and I definitely will NEVER have him as an obgyn again.

Excellent Care! Went to the Crusader Clinic at this location for a suspected ear infection. I needed to see a doctor right away and didn’t have time to make an appointment and needed an urgent care clinic that accepted IL Medicaid….

This place was perfect! They accepted my Medicaid plan and were able to see me on a walk in basis! The staff was very friendly and helpful with checking me in and the wait time to see the Nurse Practitioner ( I arrived close to the clinic’s closing time) was less than 10 minutes! Turns out that I didn’t have an ear infection after all but the Nurse Practitioner prescribed me some Zyrtec to help dry up the small fluid build up in my ear infection order to help prevent a possible infection. I was pleased with the outcome of my visit and impressed with the service! I recommend this walk in clinic if you’re in the Rockford area and need “less than ER level” medical care on short notice!
