Three Stars is because of vagueness in a written request that resulted in an unnecessary expense. We are new to Madison and scheduled an appointment with Dentistry for Madison, seeing Dr. Jay Hazen. The office has a convenient downtown location and offers parking. The staff are friendly, explain procedures and dental health in detail, and their equipment is modern. What to be aware of: Dentistry for Madison was out of our insurer's network. However, we did not anticipate the insurer would disallow payment for the full-mouth X-rays, which would cost us $416.

We appealed and discovered the reason; our insurance pays for full-mouth X-rays once every three years. We suspect that this is a reasonably standard limitation. Our full-mouth X-rays were about two years ago. The problem was with Dentistry for Madison. They emailed us with an instruction to "Please contact your previous dental office to have your most recent x-rays emailed to info@dentistryformadison. com prior to your scheduled appointment. " We did as instructed, and the dental office out of state followed the instructions.

Nowhere in the instructions did Dentistry for Madison request the most recent Full Mouth X-rays. As retired people in our 70s, we are dismayed that we incurred an unnecessary expense. It could have been avoided if the office had been more specific. We shared the issue with them and they told us they would edit the email to request the most recent Full Mouth X-rays.

I had my first visit today and it went very well! I'm usually pretty nervous at the dentist, but the staff here are so patient, knowledgeable and kind. Dr Hazen is very friendly and intelligent and explains things very well. The office is very comfortable and you can tell everyone here takes pride in their work. If you are looking for a dentist in the area, I really recommend this office!
