ESRHS operates multiple locations across the Eastern Shore, including community health centers in Franktown, Atlantic, Chincoteague Island, Eastville, and Onley, alongside several dental sites and an
Urgent Care center (Express Care in Onley). Their medical services encompass family medicine, geriatrics, internal medicine, and pediatrics, providing
Primary Care for all ages. In addition to general medical care, they offer digital X-ray, laboratory services, electronic health records, health education, assistance with prescription access, and travel immunizations. Their dental services cater to all ages, with a particular focus on children, pregnant women, high-risk patients, veterans, and the elderly. Behavioral health services include primary care consults, individual therapy, and psychiatric medication management.
As a patient-centered medical home (PCMH), ESRHS emphasizes coordinated, high-quality care that addresses both physical and mental health needs, including prevention and wellness, as well as acute and chronic care management. They are recognized for their commitment to quality and have received accolades such as being named among the top 10% nationwide for clinical performance by the federal Health Resources & Services Administration. ESRHS also participates in an Accountable Care Organization (ACO) to provide coordinated, high-quality care to Medicare patients.
