Fair Haven Community Health Care

Fair Haven Community Health Care (FHCHC) is a not-for-profit, community-based healthcare organization located in New Haven, Connecticut. Founded in 1971 by Latino activists, its mission is to provide equitable, high-quality, and culturally responsive, patient-centered care to improve the health and social well-being of the communities it serves. As a Federally Qualified Health Center (FQHC), FHCHC is dedicated to providing accessible healthcare to everyone, regardless of their financial situation, offering a sliding fee scale for uninsured patients and accepting various insurance plans, including Medicaid, Medicare, and most private insurances. FHCHC's main location is at 374 Grand Avenue in New Haven, but it has expanded to include multiple sites throughout the region, including school-based health centers. The organization provides a wide range of integrated services to cater to the diverse needs of its patients, from newborns to older adults. These services include adult primary care, pediatrics, geriatrics, midwifery and women's health, dental care, and behavioral health. Additionally, FHCHC has specialized programs for addiction medicine, HIV/AIDS services, and the Women, Infants, and Children (WIC) program. They also provide care coordination services to address social determinants of health, such as housing and food access. The organization's commitment to quality is demonstrated by its accreditation from the Joint Commission's ambulatory health care program. FHCHC has a strong history of growth and innovation, including a recent major capital project to expand its Grand Avenue campus with a new state-of-the-art facility. This expansion, which opened in June 2025, features new exam rooms, a community pharmacy, and dedicated spaces for health and wellness education, all designed to be welcoming and reflective of the neighborhood's cultural fabric.
