Stay away at all costs. I wouldn’t recommend to my worst enemy. The doctor is rarely on site, the nurse practitioner who they have serve as the acting doctor most days is completely clueless. Also, please don’t expect a medication to ever be refilled correctly or on time and always expect an attitude from the staff when you call about one. They are the worst I have ever seen treat people.

My grandpa went here after having a doctor for 40+ years that retired, he has had one arm amputated after a farm accident, had his voice box removed after having cancer, has back pain and a lot of other medical issues. . . John Cook has been so rude to my grandpa and dropped his pain medicine from 4 pills a day to 1 a day without any notice and my grandpa is very upset and in pain and had to go to the emergency room once just to get help! I will not recommend this place to anyone, the front staff is just as rude as well.

. . We went to get him a new doctor and no one wanted to take him because John Cook wrote false information about my grandpa in his file! I would put 0 but they make you use at least one star! NO ONE SHOULD GO HERE OR USE JOHN COOK AS A DOCTOR!!!!

I have no complaints about Dr. McFadden or John Cooke. I usually see John as FHN bounces Dr. McFadden around to 3 locations. John has always been professional and informative along with showing real concern for his patients.
