Hackley Community Care (HCC) is a prominent healthcare leader in Muskegon County, Michigan, founded in 1992 to address the critical need for prenatal care among low-income pregnant women. Since its humble beginnings in a small renovated house, HCC has expanded significantly, evolving into a comprehensive Patient-Centered Medical Home (PCMH) accredited by the National Committee for Quality Assurance (NCQA) at Level 3. HCC's mission is to support and promote the total health of its patients with quality, integrity, respect, and compassion. Their philosophy emphasizes serving the whole person, addressing not only medical needs but also dental, mental health, social, economic, and psychological aspects of well-being. The organization offers a wide array of services across multiple locations, including their main Baker Street site and the Clinton Street NowCare facility. Key services provided by HCC include: Primary Medical Services: Comprehensive care for all ages, from newborns to geriatrics, including preventive care (physicals, flu shots, screenings), evaluation and treatment of acute and chronic conditions, minor office surgeries, hearing and vision testing, and on-site lab services. NowCare Services: Walk-in medical and mental health services for acute, non-life-threatening conditions like colds, flu, sprains, rashes, and minor injuries. Dental Services: Expertise in dental care and oral hygiene. OB/Women's Health: Comprehensive care for pregnant mothers and women's health needs. Pharmacy Services: Community Care Pharmacy assists patients with their medication needs, including a Patient Prescription Assistance Program for those who qualify.
