Detailed Introduction

Health Partners of Western Ohio (HPWO) is a prominent non-profit, community-based organization dedicated to delivering comprehensive, integrated primary healthcare services across numerous locations in Western Ohio. Established in 2003, HPWO has grown significantly, serving a large patient population with a focus on eliminating gaps in health outcomes by ensuring access to quality, affordable, preventive, and primary healthcare for all members of the community. HPWO operates as a multi-disciplinary clinic, offering a broad spectrum of services that include medical, oral health (dental), behavioral health, pharmacy, social services & outreach, substance use treatment, and vision care. Their medical services encompass wellness and preventive care (e.g., immunizations, check-ups, health screenings), short-term sick care (e.g., sore throats, earaches), and long-term disease management (e.g., diabetes, asthma, hypertension). Oral health services include cleanings, sealants, fluoride varnishes, fillings, extractions, and dentures/partials, along with school-based programs. Behavioral health services address issues like depression, anxiety, and substance abuse, often integrating with medical care. They also provide on-site pharmacy services, offering discounts for uninsured patients and accepting various insurance plans. A cornerstone of HPWO's approach is its patient-centered, integrated model of care. This means that patients are at the core of an interprofessional healthcare team, which can include physicians, nurse practitioners, dentists, dental hygienists, social workers, counselors, and pharmacists, all collaborating to create the best treatment plan. This integrated model allows for co-located services, such as behavioral health and medical services provided in the same exam room, and even dental screenings during medical visits. HPWO is committed to affordability and accessibility. They accept Medicare, Medicaid, and most commercial insurance plans. Crucially, no one is turned away due to inability to pay, as they offer a sliding fee scale based on income and household size, as well as payment plans and financial counseling to help patients understand their options and apply for assistance. Beyond clinical care, Health Partners of Western Ohio actively engages with the community. They have opened school-based health centers, conduct outreach through social media and local groups, and provide social services that connect patients with resources for basic living needs like transportation, utilities, food, and housing. Their mission emphasizes addressing health equity and improving the overall well-being of the communities they serve.

I started going to 8th Street however due to them cancelling my appointment twice I stopped going well I needed a Dr so I decided to try the North Main office. . It was horrible the CNP talked down to me, refused to refill all my meds and none of them are even narcotics.

My power chair is missing a caster and the seat on my manual chair is ripped off half of the one side. I requested a prescription for a new manual chair. The nurse comes in with my discharge paperwork and is going over it with me I see it said walker not wheelchair I haven't been able to walk since 2018 she said she would tell her and that the CNP had probably just pushed the wrong button.

I called today and they were closed so I called quick care to see where she called it into and was told Rite Aid which made no sense but I called Rite Aid anyways just to be told that no it wasn't called in there so I called Quick Care back and was told that she didn't call it in she printed it have now been on hold for almost 20 minutes to speak with the nurse again to try and get this resolved before I end up breaking through the seat on my chair and getting hurt. . I will definitely be searching for a new Dr

Don’t take Peggy Sherman on as your provider. She has cancelled 4 appts over the last 4 mos. I finally have an appt today and she calls and wants to reschedule again. It’s important that you have a professional who will see you when you’re sick. I’m calling the BBB and turn her in for this. She is crazy to think I’d give her another chance. I found out last year I had cancer.

It took her over two and a half weeks for her to get an order for a biopsy, after many calls from myself and my cancer dr. I did have an aggressive kind of cancer. She doesn’t have your best interest in mind AT ALL. Do yourself a big favor and find someone else. Like I’m doing today. 🙏🏼
