Detailed Introduction

Heart City Health is a prominent non-profit, Federally Qualified Health Center (FQHC) established in 1993, serving the residents of Elkhart and surrounding counties in Indiana. Their core mission is to bridge healthcare gaps and ensure equitable access to high-quality, affordable healthcare for everyone, particularly focusing on underserved communities, including those who are uninsured, underinsured, or reliant on Medicaid and Medicare. Heart City Health offers a comprehensive range of services, emphasizing a "whole-person care" approach. These services include: Primary Medical Care: Covering adult and pediatric health, including chronic disease management (e.g., asthma, diabetes, heart disease, hypertension), preventative care, and geriatric medicine. Dental Services: Providing general dentistry such as examinations, cleanings, fillings, extractions, children's dentistry, and emergency appointments, often with state-of-the-art facilities. Behavioral Health: Offering counseling services for anxiety, depression, stress management, crisis intervention, individual and family counseling, and prescription management services, with options for telehealth. Pharmacy Services: An onsite pharmacy at their Simpson Street location provides convenient prescription services to patients and the community. Additional Services: They have expanded to include chiropractic care and are developing new facilities with urgent care, imaging, physical therapy, and pain management. As an FQHC, Heart City Health operates on a sliding fee scale based on income and family size, ensuring that cost is not a barrier to receiving care. They accept various insurance plans, including Medicaid, Medicare, and private insurances. Their commitment extends beyond clinical care to address social determinants of health, providing support for basic human needs like food and transportation, and engaging in community partnerships to amplify their impact. Heart City Health is recognized as a Level 3 Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA), signifying their dedication to patient-centered care and continuous quality improvement. They are actively expanding their facilities, including a new medical center in a redeveloped Sears building in Elkhart, which is projected to significantly increase their capacity to serve the community, opening in early 2026. This expansion reflects their ongoing commitment to reducing health disparities and fostering a healthier community.

Very friendly and knowledgeable with helping me understand setting up my mobile device and how to use it. Always greeting you with a smile with shows that they are concerned about you which lot of places don't do or show. I so graceful to have a team like them to be there for me.

My visit today ran more than an hour behind. My appointment was at 11:15 am, It's 12:41 and I am still waiting to be seen. I'm grateful for Dr. Wright as he came in to check on me while c waiting on Dr. Scott.

Dr. Scott has always provided great care for me and I am looking forward to seeing him today. The pharmacy staff is amazing, especially, Gene! Overall, the team at Heart City do a great job taking care of their patients.

My appointment was at 10:10 am I was not brought back until after 11 am only to wait another 20 minutes to see a nurse practitioner. I was scheduled an appointment with Angela Panford. She was extremely rude. She did not address any of my medical concerns or offer any insight to anything I brought up. She literally did anything she could to not touch or do anything physical to address any medical concerns. She told me she had other patients to see and what I wanted from the visit after all of that. When I told her she was being rude as she was walking out the door providing no help she came back in the room and proceeded to become combative.

I told her I was feeling rushed and she was not listening. She sighed and walked out of the door. I was in tears because I did have serious medical concerns. I walked out when I asked to see someone else I was told to go back to the quiet waiting room and explain my situation and people were listening and staring while I cried and tried to tell them what had happened. The reception staff got me some water and tried to schedule an appointment with someone new for a later day. Unfortunately there was a line behind me and I felt I had wasted enough time there and my trust with my health had been lost. The reception staff is very nice there.

I do not recommend this place and if you do give it a chance and are assigned to Angela Panford request someone different. She is part of the issue and why people are hesitant to visit the doctor. If I could give her a zero I would. Not a safe person in this setting what so ever.

He was friendly could see I was healthy by looking at my face He asked if I had any other concerns. He renewed my prescription with no problem. Checked my heart and lungs and I left in about 5min. Thank you Heart City Health.
