My 14 year old child was injected with numbing medicine and then denied surgery after I requested to update my address then refused the forms because it was too many papers just for an address update…… they brought out 3 separate clip boards for me to complete all because I wanted to update my address ……. . I did my best to finish forms each time an assistant brung it back being incomplete until I refused to sign the forms …….

They made a big deal about it where I felt I was being forced to fill them out ……. Now keep in mind I have wonderful insurance and never had a billing problem before and had even paid Cash for previous service had bright my children last week for service and have been seeing this dentist under a year………. .

in the middle of a prepping for procedure ona minor he just stops !!!!!!!My insurance checked out and everything ……. . …… never mind I had to report the silver hair lady in front earlier this year for being a rude mess slamming the door huffing n puffing when she could follow instructions for voluntary benefits program FLOORED AT THE TREATMENT FROM THIS OFFICE

Great place for dental care and to work. 🥰🥰🥰. Even my kids love it here! If you are a consumer with questions like Miss Kristy D I am happy to answer them. Signing the paperwork at a doctor’s office, especially for work on your child, is your consent to treatment. Most of us would not want our kids treated without our permission.

Your signature on your form is a legal document giving your permission, without it it’s illegal for providers to continue. It sounds like this staff made the most ethical decision to protect you and your daughter from services you were unwilling to consent for. In the future if anyone has needs that limit their ability to read or write on our reasonable and customary dental office forms please ask for assistance. Your signature however is required as your permission.

Been going to Dr. Stephen and Justin Krawiec my whole life for my dental needs. First with Dr. Stephen Krawiec and then his Son Dr. Justin Krawiec when he joined the practice many years ago.

They have great bedside manner, thoroughly explain any issues going on in the mouth, and are gentle when operating in sensitive spots of the mouth. Recommend to anyone near and far. I drive 45 minutes each way because I trust them 100% with all my dental needs.
