Lakeshore Community Health Care (LCHC) is a 501(c)(3) non-profit organization operating as a Federally Qualified Health Center (FQHC). Located at 1721 Saemann Ave, Sheboygan, WI 53081, with multiple locations across Wisconsin (including Manitowoc and West Bend), LCHC is committed to improving access to healthcare for all individuals in its service areas, particularly those who are underserved or face barriers to care. They also offer a school-based mental health program to provide therapy to students during the school day, removing transportation and financial barriers. Dental Care: General dentistry services such as routine exams, cleanings, fluoride treatment, sealants, dentures, fillings, root canals, and emergency dental care. LCHC also runs a school dental program. Chiropractic Services: Available to address various musculoskeletal conditions. Pharmacy Services: Providing prescription refills, transfers, personalized medication packaging, and medication monitoring. Importantly, they offer sliding scale pricing for uninsured patients and have resources to ensure patients receive necessary medications even if they cannot afford them. A core tenet of LCHC's mission is to eliminate health disparities among the underserved. They accept various insurance plans including Medicaid, Medicare, and private insurance, and importantly, provide care to uninsured patients through a sliding scale fee system based on household income. They also provide educational programs on diet, nutrition, and smoking cessation. They also assist with transportation arrangements when needed.
