Located at 1721 Saemann Ave, Sheboygan, WI 53081, with multiple locations across Wisconsin (including Manitowoc and West Bend), LCHC is committed to improving access to healthcare for all individuals in its service areas, particularly those who are underserved or face barriers to care. LCHC functions as a patient-centered medical home, offering a wide array of integrated health services under one roof. These services include: Primary and Preventive Medical Care: Annual wellness exams, physical exams for school or work, chronic disease management, acute care for common illnesses, health screenings, and immunizations. Behavioral (Mental) Health Services: Counseling, mental health screenings, and integrated care with medical teams to address conditions like anxiety, depression, PTSD, and substance abuse. They also offer a school-based mental health program to provide therapy to students during the school day, removing transportation and financial barriers. Dental Care: General dentistry services such as routine exams, cleanings, fluoride treatment, sealants, dentures, fillings, root canals, and emergency dental care. LCHC also runs a school dental program. Chiropractic Services: Available to address various musculoskeletal conditions. Pharmacy Services: Providing prescription refills, transfers, personalized medication packaging, and medication monitoring. Importantly, they offer sliding scale pricing for uninsured patients and have resources to ensure patients receive necessary medications even if they cannot afford them. A core tenet of LCHC's mission is to eliminate health disparities among the underserved. They accept various insurance plans including Medicaid, Medicare, and private insurance, and importantly, provide care to uninsured patients through a sliding scale fee system based on household income. No patient is turned away due to inability to pay. Beyond direct medical services, LCHC offers robust support services to address social determinants of health. Their care managers assist patients with insurance enrollment, connect them to community resources for housing, healthy food, employment, and other vital needs. They also provide educational programs on diet, nutrition, and smoking cessation. LCHC is committed to providing culturally competent care, offering interpretation services in all languages and having Spanish and Hmong-speaking staff on-site. They also assist with transportation arrangements when needed. Lakeshore Community Health Care prides itself on a team-based approach, where medical, dental, and mental health professionals, along with pharmacists and case managers, collaborate to create personalized health plans for each patient, ensuring a holistic understanding and treatment of their unique needs. The organization was established in 2009 and has grown to serve thousands of unduplicated patients annually across its various programs. Patient testimonials highlight the convenience of integrated services, the compassionate and skilled staff, and the affordability of care, particularly for those with limited income or insurance.

Was a previous patient with this hole, got maybe 3 fillings with them, (Over almost 3-4 months) all of them have chipped in just under a year, they never finished crowns, they had horrendous wait times, availability was horrendous if you even got in and not canceled on, only state approved place in town, and it shows that they care so little about their patients and servicing the Public. They need more staff, another clinic even. It's ridiculous to see all the sad stories from this place, but that seems to be all too common nowadays.

Do better.

(Harassment/unprofessional behavior/ Racism) Took my two son for a same day dental appointments with Hygienist Heather M. While Heather was examined my older son, I went back to checked my younger son in for his next appointment. I came back and Heather did not explain what’s procedure she was doing. My younger son and I waited on the hallway. I saw my older son kept throwing up and not comfortable because Heather kept inserting the tool in his mouth multiple times.

She was upset the tool didn’t go in and kept pushing the tool in with an attitude. I went in the room to comfort my child and Heather yelled and pointed finger at my face OUT! OUT RIGHT NOW! I did not try to argue with an unprofessional person so I left the room. I was harassed by a racism and unprofessional Hygienist Heather M. Who shouldn’t be working at a community services. Since that day, my children had bad experience of trauma from Lakeshore Community Healthcare providers.

I felt unsafe for children who were treated by a bad behavior providers. I highly don’t recommend Lakeshore Community Healthcare to provide care for school systems due to their Lack of knowledge and unprofessional behavior period Know that you did not earn a STAR for your service but for myself to post this review for our children safety.
