Detailed Introduction

Lakeshore Community Health Care (LCHC) is a prominent 501(c)(3) non-profit organization dedicated to improving community health by providing accessible and integrated healthcare services. Established in 2012, LCHC operates as a federally qualified health center (FQHC), meaning they provide care to patients regardless of their ability to pay, utilizing a sliding fee scale based on income and family size to make services affordable. Their comprehensive approach to healthcare encompasses primary medical care, dental services, behavioral (mental) health, chiropractic care, and pharmacy services. LCHC is recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance, highlighting their commitment to high-quality, integrated care and patient satisfaction. They aim to address health disparities among underserved populations by providing holistic "whole-person" care, where medical and mental health providers work collaboratively to achieve patient health goals. LCHC offers a wide range of medical services, including annual wellness exams, physicals, chronic disease management, acute care for common illnesses, health screenings, and immunizations. Their dental services cover preventive care, cleanings, and other general dental needs. Behavioral health services include counseling, therapy, and psychiatric support, often integrated with primary care to address the interconnectedness of physical and mental well-being. Lakeshore Community Health Care has multiple locations across Sheboygan, Manitowoc, and Washington Counties in Wisconsin. They also have robust community outreach programs, including school-based mental health services through initiatives like the Providing Access to Healing (PATH) program, which aims to support students within the school day and remove barriers to accessing care. LCHC is committed to utilizing technology, offering telehealth services for virtual consultations and remote monitoring, and providing an online patient portal (Lakeshore Health Connect/MyChart) for convenient access to health records and communication with providers. The organization's mission is rooted in being responsive to community needs and ensuring everyone has a right to quality healthcare, regardless of their insurance status or income level. They employ a team-based approach, with doctors, dentists, mental health professionals, pharmacists, and case managers collaborating to create personalized health plans for each patient. Case managers also connect patients to essential community resources such as housing, healthy food, employment, and insurance, recognizing these factors' impact on overall health.

Everyone is always kind, caring, and thorough with my care. They treat me like a human, not lust another number like some bigger chain offices. I was able to get all my treatment completed in a timely manner and all my concerns were met. When my 2 year old had his first appointment a few months back he was really scared and didn't want to come.

But after a few minutes he was calmed down and happy. Now he asks when he can come back because he likes the dentist. I would recommend them to anyone that is looking for dental care.

I’ve had horrible dental care in the past. I came to this clinic terrified, absolutely traumatized. I cried just talking about my past experiences. Dr. Josh and his team were patient, they listened, they comforted me, and they remembered our discussions several appointments later. One of my biggest concerns was that I had a past dentist not stop when I was expressing pain. Dr.

Josh kept assuring me if I needed a break, if I was in pain, if anything was off that he would stop immediately and help me. He took the time to tell me what he was doing with each step. He even tried his best to talk to his team about random things on the spot because I asked for distractions. I felt safe. I felt heard. I felt respected. I don’t plan on seeing any other dental staff ever.

They are incredible and I hope they see this. I am so so grateful. I’ve been unfortunate to have met a lot of cruel people in my life and I cannot express how much it means to me that you all were so kind and thoughtful. You made a difference. Thank you so much.

I would not recommend this place if you are struggling with mental health. The provider I saw was only willing to prescribe me the exact same medications I had been on before and I had told her did not work for me. I felt very judged. The counselors are not very good. When I finally was willing to accept the medication the provider prescribed, she wouldn't prescribe it to me anymore, and I wound up hospitalized for a week.

To me, it just seems like they don't know what they're doing here. If you're struggling with mental health, I recommend CFS instead. They got me the correct diagnosis and I was able to get better quickly.

I have a broken tooth from a domestic violence incident that is now abscessed and infected the whole side of my face I had to go to the er to be put on antibiotics and called the next business day I spoke to triage and made an appointment for the next day. Got there waited an hour and a half to finally see the dentist he quickly looked at it and said they can fix it in a few months when the waitlist opens up. I literally cannot wait a few months the abscess will probably turn septic by that point.

Definitely don’t recommend. If I die from the infection at least my family will be able to sue and have this place shut down 🤷‍♀️
