Mosaic Health Lyons

Mosaic Health is a healthcare organization with a long history of providing comprehensive and accessible health services. Founded in Rochester, New York, in 1964 as Neighborhood Health Centers of Monroe County (NHCMC), the organization has expanded its reach over the decades, now operating health centers throughout New York State, from the Finger Lakes to the Mohawk Valley. In 2019, it was renamed Mosaic Health to better reflect the diverse communities it serves. The organization is a federally qualified health center (FQHC) and is committed to a mission of providing compassionate, individualized health care and wellness-related education to all, regardless of financial, cultural, or social barriers. The Mosaic Health Lyons location, situated at 1519 Nye Rd, Lyons, NY 14489, was once a site for dental services. However, in mid-2024, Mosaic Health announced the closure of its dental offices in Lyons and Mt. Morris. This decision was a direct result of a significant reduction in grant revenue and the ongoing national shortage of dentists, which made it particularly difficult to recruit and retain dental professionals in rural areas. Despite the closure of the dental office in Lyons, patients who were in the middle of a treatment plan were offered the option to continue their care at other Mosaic Health locations, such as the ones in Rushville or Utica. Mosaic Health's broader network of clinics continues to offer a wide array of services. These include comprehensive medical care (such as family medicine, well-child visits, women's health, and chronic disease management), behavioral health services, and dental care at its remaining dental offices. The organization also provides integrated care for conditions like HIV and Hepatitis C, and offers a sliding fee discount program for uninsured or under-insured patients to ensure quality healthcare remains affordable. Additionally, Mosaic Health has a mobile unit to further expand its reach and a team of staff dedicated to helping patients access social programs and other community resources.
