The UK North Fork Valley Community Health Center (NFVCHC) is a comprehensive healthcare facility located at 750 Morton Blvd, Hazard, KY 41701. It operates as a joint venture between the University of Kentucky College of Medicine and the North Fork Valley Community Health Center board, making it the first university-affiliated community health center in Kentucky with a family medicine residency training program. As a Federally Qualified Health Center (FQHC), NFVCHC is committed to providing accessible and affordable primary healthcare and related services to the residents of Perry, Knott, Letcher, Harlan, Breathitt, Leslie, Clay, and Owsley counties, and surrounding areas in Southeastern Kentucky. The center embraces a patient-centric approach, offering care to all individuals regardless of their income or ability to pay, with services available on a sliding fee scale based on income and family size. The extensive range of services offered at UK North Fork Valley Community Health Center includes: Primary Care: Comprehensive care for patients of all ages, including annual wellness visits, physical examinations, family planning, medication management, preventive screenings (e.g., Pap tests, PSA blood tests), and vaccinations for children and adults. Acute Illness/Injury/Minor Emergency Service: Diagnosis and treatment of common short-term illnesses like colds and flu. Chronic Disease Management: Care for chronic conditions, geriatric care, and testing/monitoring of blood pressure, blood sugar, and cholesterol. Dental Care: Full-service dental care for all ages, available by appointment and walk-in, including preventative care, general dentistry, and potentially mobile clinic services. Medicaid and most dental insurances are accepted. Behavioral and Psychological Health Services: Services from licensed psychologists to address mental and emotional well-being. Pharmacy Services: An in-house pharmacy offering easy refills, medication consultations, and a discount pharmacy program for eligible patients. Laboratory Services: On-site lab testing with often next-day results. Preventive Medicine/Health Education: Health education services and nutritional counseling. Specialized Screenings: Pre-employment, DOT, and school exams. Osteopathic Manipulative Treatment: A hands-on treatment approach for pain and other conditions. Workers' Compensation: Services for work-related injuries. UK North Fork Valley Community Health Center has received significant recognition for its quality of care. In 2024, it earned six Health Resources and Services Administration (HRSA) quality awards, distinguishing itself as one of only six similar health centers nationwide to receive the National Quality Leader in Diabetes Health award. It was also among only 26 to earn the National Quality Leader in Heart Health award and was recognized as a Gold Health Center Quality Leader for overall quality measures, placing it in the top 10% of all health centers.
