Kentucky Mountain Health Alliance, Inc. (KMHA) is a private, non-profit organization established in 2005. Its primary mission is to improve the health and well-being of underserved citizens in Southeastern Kentucky, particularly those who are low-income, uninsured, underinsured, or experiencing homelessness or at risk of homelessness. Operating out of its permanent service delivery site, the Little Flower Clinic located at 279 East Main Street, Hazard, KY, KMHA serves Perry County and the surrounding areas. KMHA takes a holistic approach to healthcare, addressing physical, spiritual, mental, cultural, emotional, and social well-being. They strive to provide quality medical care, oral health care, and behavioral health services. Their comprehensive services include, but are not limited to, primary care, family medicine, internal medicine, dentistry (cleanings, fillings, extractions, root canals, crowns, bridges, dentures, and oral cancer screenings), mental health services, Vivitrol injections, Retinavue eye testing, onsite X-ray and laboratory services, chronic disease/case management, health education, and transportation for active patients. They also offer referrals for specialty and inpatient care. A key aspect of KMHA's commitment to accessible healthcare is its participation in the 340B Drug Pricing Program, which allows them to provide outpatient drugs at significantly reduced prices to eligible patients. They also offer a sliding fee scale for uninsured patients, based on gross income and family size, adhering to federal poverty guidelines. Little Flower Clinic has been recognized as a Patient-Centered Medical Home (PCMH) since 2015 and achieved a distinction in Behavioral Health Integration in 2023. This recognition underscores their dedication to providing integrated care that addresses both physical and behavioral health needs, aiming to reduce barriers to services and improve overall patient outcomes. KMHA actively collaborates with various local, state, and federal agencies, as well as community partners, to achieve its goals and serve the persistently impoverished and historically underserved population of Southeastern Kentucky. They also utilize a mobile medical clinic to reach individuals in more remote areas within Perry County.
