Detailed Introduction

Practice Without Pressure (PWP) Pike Creek is a dedicated non-profit organization located at 3105 Limestone Rd, Wilmington, DE 19808. It is a clinic that provides essential dental, medical, and personal care services with a strong focus on individuals with disabilities and those who experience significant anxiety, fear, or resistance to traditional healthcare procedures. The cornerstone of PWP Pike Creek's approach is its innovative "Practice Model™." This model, developed by Deborah Jastrebski for her son with Down Syndrome and complex medical needs, is a step-by-step methodology designed to help patients gradually become comfortable and familiar with procedures they once feared or avoided. Instead of relying on sedation or physical restraint, the PWP Model emphasizes building trust and learning through individualized sessions with "practice specialists." These specialists work with the individual, their family/caregivers, and clinicians to create a supportive environment where challenging routines are broken down into manageable steps, fostering confidence and self-management skills. PWP Pike Creek offers a comprehensive range of services, including routine dental care (cleanings, exams, X-rays, fillings, root canals, extractions, dentures), medical care (women's health exams, blood draws, pre-surgery support), and personal care (haircuts, nail care, toothbrushing instructions). Their commitment extends to making personalized service routine, with staff of dental and medical professionals trained to meet diverse needs. Beyond direct patient care, PWP also engages in broader initiatives. They offer "Treasure Your Health," a breast awareness program for women with disabilities and caregivers, and "oral health in the Home," a fee-based training program for families and caregivers to improve daily dental care for individuals with disabilities at home. PWP has gained national recognition for its model of care delivery, with its philosophy and model presented to hundreds of clinicians nationally and regionally. They accept Medicare, Medicaid, and most major medical insurance for covered treatments upon review.

I was very unhappy here. First, I transferred from another location because they told me this was the only office my new insurance covered. Then I didn't get my regular dentist, which was the reason I purchased that insurance to begin with.

The new dentist told my son and I BOTH would need extensive fillings and I would need a crown on a tooth that did NOT have a cavity due to a low filling near gum line. Rather than do the work there, I transferred to a new dentist who did not recommend ANY of that work! Upon leaving they started billing me $50 each for my son and I for LEAVING THE PRACTICE after one visit which was a cleaning and x-rays paid for by insurance. DO NOT go here!

I could not be more disappointed with my experience at this office. My son with special needs was in need of someone to fix a cavity, which his current pediatric dentist could not complete. When I spoke with someone at the office initially, I was told about a service they offered called a patient advocate. They told me my insurance did not cover this service, and I said I was not interested at this time, but maybe in the future. When they called to confirm the appointment, they said they could probably complete the work the same day. This was a blatant lie. When we arrived for our appointment we were taken aback at the condition of the room we were placed in. It was dirty, dilapidated, and the counter tops in this room were covered with junk.

My son was placed in a regular chair, not an exam chair. The hygienist was very pleasant, however, the dentist repeatedly spoke to her with intense frustration as she retrieved the wrong toothpaste and was unable to close a door that appeared to be stuck. When my son was sitting in the chair, the dentist handed him a card that said "stop" without knowing anything about his abilities and began trying to get him to open his mouth while sitting straight up in the chair. After a 10 minute explanation of the theory behind the practice and explaining, twice, about the patient advocate service (to which I declined, twice, again) the dentist called in the patient advocate. I had to literally say STOP to prevent her from billing me for this service. I was in desperate need to address the problem at hand, which was my son's cavity, which had been festering for weeks due to my inability to find a dentist who would take him due to COVID. When we said we were again, not interested, the dentist and patient advocate were clearly offended and the patient advocate left the room abruptly. Clearly upset, the dentist then began to try again to look in his mouth with force, going against her entire earlier explanation of the goal of the practice.

My son, obviously, was not cooperating and closed his mouth on her finger, which she then proceeded to complain about. Without a good look, she proclaimed he had 5-6 cavities, needed extensive work including root canals and extractions, and told us it would be a while before we could be scheduled for the procedure (when we took him to another dentist, he said he only saw two small cavities and placed caps on them as they are baby teeth). Needless to say, my husband and I were so shocked at the demeanor and repeated attempts to clearly extensively bill us, that we left there very upset. We then received a bill in the mail for a "Comprehensive Dental Exam" which couldn't have been farther from the truth AND they tried to over charge us beyond what our insurance company told us we owed. Please be aware of this dentist and her practices! It is a shame that someone that markets themselves as providing an extra level of care to those with disabilities is in fact trying to take advantage of some individuals who need it the most.

We needed to get my 19 yo, severely autistic son a TB test. We went to 2 other testing sites and they couldn’t do the blood draw or the skin test. But Josephine got it done! She was amazing and I’m so grateful!
