Detailed Introduction

Small Smiles Dental Center: Croom Olivia DDS, located at 4257 W Third St, Dayton, OH 45417, was a part of the broader "Small Smiles Dental Centers" chain. This chain was a privately-owned network of dental clinics across the United States, established with a stated focus on serving children, particularly those from low-income families and those covered by Medicaid. The original Small Smiles practice reportedly began in 1928 in Pueblo, Colorado, founded by Bruno DeRose, and later expanded significantly to multiple states. The Dayton, OH location, like others in the chain, aimed to provide general dentistry services. However, the Small Smiles Dental Centers as a whole became widely known for extensive controversies and legal issues. These included allegations of performing unnecessary dental procedures on children, often without proper anesthesia or parental consent, and engaging in fraudulent billing practices targeting Medicaid. Reports and investigations highlighted instances of excessive use of restraint devices (like papoose boards) and a focus on high volume procedures to maximize profit, leading to a $43 million global settlement in a class-action lawsuit. The chain's parent company, Church Street Health Management (CSHM), ultimately filed for bankruptcy. While the specific role of Dr. Olivia Croom, DDS, at this particular location is not extensively detailed in the public records regarding the controversies, her name is associated with the Dayton clinic. Dr. Croom has a significant career in dentistry, having pursued further education and held leadership positions in other oral health organizations like DentaQuest. It's important to distinguish between the actions of the larger corporate entity (Small Smiles Dental Centers) and individual practitioners who may have worked there. The overall reputation of the Small Smiles Dental Centers chain is heavily impacted by the widespread legal and ethical issues it faced.

Just got done at this dentist office. He is all about the billing. I'm glad that the one nurse stepped up and said no. He was trying to schedule my nephew for things that they are not equipped to handle.

He said that my 3 year old son needs his tooth pulled, which he might but I'm getting a second opinion on this given the circumstances. He also kept trying to get both boys to say that they are beaten, including the 3 year old. Definitely will not be going back.

I had not been back to this office in a long while, but when I returned to their office again last week for my son and daughter, they were able to service my 11 year old daughter, but not my 16 year old son because they stated that he would need "adult equipment" to be able to do what he needed to get done. They did give me a paper with referrals and I called a dentist office that would see him but they needed a referral from small smiles office in order to be seen quicker than April 2025. So, I called small smiles back and they were nice about it and said they would send the referral right then and there. . . . . I called this morning (the other dentist office) and they looked up my son's name and they did not see a referral in their system (after I was told it was sent by small smiles associate).

I called small smiles back only to learn and see that they are only open on Mondays and Wednesdays. It's just too much hoop jumping and a little irritating. I had been going to small smiles since my now 22 year old was a child and it most definitely isn't the same anymore. You can't even go back with your child anymore because they have had other ignorant parents try and fight the staff. This world is. . . .

. . I'm just tired of doing everything I'm asked but still have to jump through hoops in order to get and keep my children's dental health up. . . . . smh
